Website Terms and Conditions
Thank you for your interest in ContactNurse ("contactnurse," "we," or "us") and our website located at www.contactnurse.com, including any associated subdomains (collectively, the "Site"). These Terms and Conditions ("Terms"), along with our Privacy Policy, govern your access to and use of the Site.
Note: These Terms govern the Site only. Your use of the ContactNurse mobile app is governed by the App Terms of Use below, and our handling of information is described in our Privacy Policy. Advisory services, if any, are subject to separate service agreements.
1. Use License
Subject to your compliance with these Terms, we grant you a non-exclusive, non-transferable, revocable, limited license to access and use the Site for informational and personal purposes only. You may not:
- Modify or copy the materials.
- Use the materials for any commercial purpose without written permission.
- Attempt to reverse engineer any software on the Site.
- Bypass or interfere with security features.
All intellectual property rights in the Site and its contents belong to contactnurse or its licensors.
2. Feedback
If you provide us with feedback or suggestions about the Site, you grant us a perpetual, irrevocable, royalty-free license to use that feedback for any purpose.
3. Third-Party Content
The Site may include third-party content or links to third-party sites. contactnurse does not control and is not responsible for any third-party content.
4. Monitoring
We may, but are not obligated to, monitor activity on the Site. Any monitoring will be conducted in accordance with our Privacy Policy.
5. Termination
We reserve the right to suspend or terminate your access to the Site at any time, with or without cause. Upon termination, all rights granted to you will immediately cease.
6. Indemnification
You agree to indemnify and hold harmless contactnurse and its officers, employees, and affiliates from any claim arising from your use of the Site, violation of these Terms, or infringement of third-party rights.
7. Disclaimer
The Site is provided "as is" without warranties of any kind. contactnurse disclaims all implied warranties, including merchantability, fitness for a particular purpose, and non-infringement.
8. Limitation of Liability
To the fullest extent permitted by law, contactnurse will not be liable for any indirect, incidental, or consequential damages arising from your use of the Site. Our total liability is limited to $100.
9. Changes to Terms
We may update these Terms at any time. If we make material changes, we'll notify you. Continued use of the Site means you accept the revised Terms.

These Terms are governed by the laws of the State of Colorado, without regard to conflict of law principles.

App Terms of Use
These App Terms of Use ("App Terms") govern your use of the ContactNurse mobile application for iPhone (the "App"), provided by Healthzum, LLC ("we," "us"). By downloading or using the App, you agree to these App Terms. If you do not agree, do not use the App. These App Terms incorporate our Privacy Policy. Where these App Terms conflict with Apple's standard licensed application end user license agreement, these App Terms control to the extent permitted.
1. Not Medical Advice — Emergencies
The App is a navigation and information tool only. It does not provide medical advice, diagnosis, or treatment, and it does not create a provider–patient relationship. Content in the App — including nurse line listings, provider search results, care categories, cost estimates, and care profiles — is for general informational purposes only. Always seek the advice of a qualified healthcare professional with any questions about a medical condition, and never disregard or delay professional medical advice because of something you saw in the App.
If you are experiencing a medical emergency, call 911 (or your local emergency number) immediately. Do not rely on the App to summon emergency services.
2. Accuracy of Information
Phone numbers, availability, provider listings, facility locations, program eligibility, and cost estimates in the App come from public sources, third parties, and bundled reference data, and may be incomplete, outdated, or incorrect. Cost estimates are rough national ranges, not quotes. Care profiles describe typical plan or program resources — your specific benefits may differ. Always verify with your member ID card, your plan's official materials, or the provider directly. We are not responsible for the acts, omissions, availability, or quality of any provider, nurse line, telehealth service, or other third party reachable through the App.
3. License and Acceptable Use
We grant you a personal, non-exclusive, non-transferable, revocable license to use the App on Apple devices you own or control, as permitted by the App Store terms. You may not reverse engineer the App, use it to build a competing dataset or service, interfere with its operation or security, or use it in any unlawful manner.
4. Partner and Employer Programs
If you join a partner or employer program through the App, the program's personalization and optional reporting are described in our Privacy Policy. Program availability is controlled by the sponsoring organization, and we may modify or discontinue a program at any time. Your participation is voluntary and you may leave a program at any time in Settings.
5. Disclaimer of Warranties
THE APP IS PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, ACCURACY, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE APP WILL BE UNINTERRUPTED, ERROR-FREE, OR THAT ANY INFORMATION IN IT IS CURRENT OR ACCURATE.
6. Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY LAW, HEALTHZUM, LLC AND ITS OFFICERS, EMPLOYEES, AND AFFILIATES WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR FOR ANY PERSONAL INJURY, ARISING FROM OR RELATED TO YOUR USE OF THE APP OR YOUR RELIANCE ON ANY INFORMATION IN IT, INCLUDING YOUR INTERACTIONS WITH ANY THIRD-PARTY PROVIDER OR SERVICE REACHED THROUGH THE APP. OUR TOTAL AGGREGATE LIABILITY FOR ALL CLAIMS RELATING TO THE APP IS LIMITED TO ONE HUNDRED DOLLARS ($100). SOME JURISDICTIONS DO NOT ALLOW CERTAIN LIMITATIONS, SO SOME OF THESE LIMITATIONS MAY NOT APPLY TO YOU. NOTHING IN THESE APP TERMS LIMITS LIABILITY THAT CANNOT BE LIMITED UNDER APPLICABLE LAW.
7. Indemnification
You agree to indemnify and hold harmless Healthzum, LLC and its officers, employees, and affiliates from any claim arising from your violation of these App Terms or your misuse of the App.
8. Changes and Termination
We may update the App, these App Terms, or discontinue the App at any time. Material changes to these App Terms will be posted on this page with an updated date; continued use of the App after changes take effect constitutes acceptance. We may suspend or terminate your access for violation of these App Terms.
9. Governing Law
These App Terms are governed by the laws of the State of Colorado, without regard to conflict of law principles. Any dispute that is not subject to a mandatory forum under applicable law will be brought in the state or federal courts located in Colorado.

Privacy Policy
Healthzum, LLC ("ContactNurse," "we," or "us") is committed to protecting your privacy. This Privacy Policy explains how we handle information when you use our website at www.contactnurse.com and the ContactNurse mobile app for iPhone (collectively, our "Services").
ContactNurse is a healthcare navigation tool. We do not provide medical advice, diagnosis, or treatment. We do not require you to create an account to use the app.
HIPAA notice. ContactNurse is not a healthcare provider, health plan, or healthcare clearinghouse, and Healthzum, LLC is not a "covered entity" or "business associate" under the Health Insurance Portability and Accountability Act (HIPAA). Information handled by the Services is not "protected health information" under HIPAA. This Privacy Policy — not HIPAA — describes how we handle information.
Our Services are operated in the United States, and information we handle is processed and stored in the United States.
ContactNurse Mobile App (iOS)
The app is designed so your everyday preferences stay on your device. The sections below describe what the app stores locally, what it uses temporarily, and when limited information may be sent over the internet.
What we do not collect
ContactNurse does not ask for or collect:
- Your name, email address, or account credentials (there is no sign-in).
- Member ID, employee ID, Social Security number, or insurance policy number.
- Diagnoses, symptoms, medications, or the content of phone calls you make from the app.
- Your location history or continuous location tracking.
- Audio recordings of your voice.
We do not sell your personal information. We do not use app data for cross-app advertising or tracking.
Information stored on your device
The app saves preferences locally on your iPhone using standard on-device storage (for example, your selected care profile, My Team saved nurse lines and providers (physicians, dentists, pharmacy), favorite nurse lines, a custom nurse number from your ID card, onboarding choices, and whether to show the triage screen). This information remains on your device unless you delete the app or clear it in Settings.
If you open ContactNurse from a partner or employer benefits link, the app may also store on your device:
- Which organization link opened the app (for example, a health plan or employer program name).
- Referrer or campaign tags included in the link (for example,
sourceorcampaignparameters). - Eligibility context included in the link (for example, member or employee), when provided.
- Whether the link was cryptographically signed and verified.
This partner context personalizes shortcuts in the app. For most partner links, it stays on your device only. See Partner and employer programs below for optional reporting.
Information used but not stored by ContactNurse
- Location: If you allow location access, the app uses your location only while it is open to show nearby urgent care, hospitals, pharmacies, labs, and similar listings (for example, through Apple Maps). ContactNurse does not store your location or share it with us for marketing.
- Microphone and speech: If you use voice features ("Ask"), the app uses Apple's speech recognition to understand your request. Recognition may occur on-device when supported, or through Apple's services otherwise. ContactNurse does not record or store your audio.
Information sent over the internet
When you use certain features, your device may communicate with services outside the app, including:
- Apple Maps and related location queries when you search for nearby care or providers.
- Public U.S. provider directories (for example, the NPI Registry) when you search for physicians or other licensed providers.
- Third-party websites and phone systems when you tap outbound links (for example, a health plan telehealth page, crisis line, or insurer nurse line). Those services have their own privacy practices.
These requests are initiated by actions you take in the app. ContactNurse does not receive your full browsing history on those external sites.
Partner and employer programs (optional)
Some employers and health organizations may invite you to ContactNurse through a personalized link
(including universal links such as contactnurse.com/go/…). When you open such a link, the
app can apply a care profile with your nurse line, telehealth, and benefits shortcuts.
Pilot programs with consent. Some partner programs ask for your explicit consent before enabling de-identified navigation reporting to help the organization measure whether the program is useful. If you decline, nothing is sent to us or to the sponsoring organization — no decline event, no link data, nothing — and ContactNurse continues as the standard app without partner reporting.
If you join a program that supports reporting, we may send navigation events to ContactNurse servers. Each event may include:
- That you opened the app from a partner link.
- That you opened a category such as nurse line, telehealth, urgent care, or mental health resources.
- The partner program ID and any campaign, referrer, and eligibility tags (for example, member or employee) associated with the link you used.
- A timestamp and a randomly generated event identifier.
We do not include in these events:
- Your name, member ID, employee ID, phone number, or email.
- Your location, favorites, custom nurse number, or care profile selections as identifiable records.
- Diagnoses, symptoms, call content, or which specific provider you called.
What sponsors receive. We share reports with the organization that sponsors your program in aggregated, de-identified form only (for example, "nurse line opened 24 times this month"). We do not give sponsoring organizations event-level data about identifiable individuals, and our agreements with sponsors prohibit attempts to re-identify individuals.
IP addresses. Like any internet transmission, uploads expose your device's IP address to our servers. We do not store IP addresses with navigation events, do not include them in partner reports, and do not use them to identify you.
Retention. Uploaded events are retained for the duration of the partner program and no longer than 24 months, after which they are deleted or kept only in aggregated form that does not identify any individual. A limited queue of recent events may also be stored on your device until it is successfully uploaded.
Leaving a program. When you leave a partner program in Settings, events recorded while you were enrolled — including the fact that you left — are delivered in a final upload, and no new events are recorded or sent after that. Navigation events are not used for cross-app tracking or advertising.
Your choices in the app
- Change or clear your care profile anytime in Settings.
- Decline a partner pilot program when asked for consent.
- Leave program (for enrolled pilot programs) or Clear partner context to remove partner attribution. After a final upload of events recorded while you were enrolled, no further events are recorded or sent.
- Revoke location or microphone permission in iPhone Settings at any time.
- Delete the app to remove locally stored preferences.
Children
ContactNurse is not directed at children under 13, and we do not knowingly collect personal information from children. The app may be used by caregivers to find legitimate care resources. If you believe we have received information about a child, contact us and we will take appropriate steps.
Consumer Health Data
This section supplements the rest of this Privacy Policy and applies to "consumer health data" as defined by the Washington My Health My Data Act, the Nevada consumer health data law, and similar state laws — including information that could identify your attempt to seek health care services.
Categories of consumer health data we may collect
- Navigation events indicating that a category of care resource was opened in the app (for example, nurse line, telehealth, urgent care, or mental health resources) — only if you have joined a partner program with reporting and given consent as described in Partner and employer programs.
- Care profile and partner link context stored on your device (which does not leave your device outside a consented program).
We do not collect biometric data, precise location history, diagnoses, treatments, prescriptions, reproductive or sexual health information, or gender-affirming care information. Browsing resource pages in the app without a consented partner program does not send consumer health data to us.
Sources and purposes
Consumer health data comes only from your own use of the app. We collect it solely to produce aggregated, de-identified program reports for the sponsoring organization, as described above. We do not use it for advertising and we do not sell it.
Sharing
We share consumer health data only (i) in aggregated, de-identified form with the organization sponsoring your program, (ii) with service providers that host our infrastructure under contractual confidentiality obligations, or (iii) as described under When we may disclose information.
Your rights
- Withdraw consent at any time by leaving the partner program in Settings.
- Access a copy of the navigation events currently stored on your device (visible in Settings under Partner entry).
- Deletion: request deletion of consumer health data we hold by emailing legal@contactnurse.com. We will respond within the time required by applicable law and will also instruct our service providers to delete it.
- Appeal: if we decline a request, you may appeal by replying to our decision; we will respond to appeals within the period required by applicable law. Washington residents may also contact the Washington Attorney General.
We will not discriminate against you for exercising any of these rights.

This section applies when you visit our website, not when you use the mobile app features described above.
Information we may collect on the website
- Personal information: Name, email address, or company information when you contact us.
- Usage data: Pages visited, time spent, and general interaction with the Site.
- Technical data: IP address, browser type, device information, and cookies.
How we use website information
- Provide and improve our website and respond to inquiries.
- Send updates or marketing communications where you have opted in.
- Analyze website usage to enhance user experience.
- Comply with legal obligations.
Cookies and analytics on the website
We use cookies and similar technologies to enhance browsing and understand website traffic. You can control cookies through your browser preferences. Our website may use third-party analytics tools (such as Google Analytics) subject to those providers' policies.
When we may disclose information
Regardless of the sections above, we may disclose information we hold:
- Legal process: when required by law, subpoena, court order, or other legal process, or to respond to lawful requests from public authorities.
- Safety and protection: when we believe in good faith that disclosure is necessary to protect the rights, property, or safety of our users, the public, or Healthzum, LLC, including to investigate fraud or security incidents.
- Business transfers: in connection with a merger, acquisition, financing, or sale of all or part of our business or assets. If information is transferred, it remains subject to this Privacy Policy (or a successor policy with at least comparable protections), and we will provide notice of any material change in how it is handled.
- Service providers: to vendors that host or operate our infrastructure on our behalf, under contracts that limit their use of the information to providing services to us.
Data security
We implement reasonable technical and organizational measures to protect information we handle, including encryption in transit (TLS) for data sent to our servers. However, no method of transmission over the internet or electronic storage is completely secure.
Third-party services
Our Services may link to third-party websites, apps, maps, directories, crisis lines, and health plan resources. We do not control those services and are not responsible for their privacy practices. We encourage you to review their policies before use.
Your rights
Depending on where you live, you may have the right to:
- Access or update personal information we hold about you (primarily from website contact forms or email).
- Request deletion of information we store about you.
- Opt out of marketing communications.
- Request a copy of your data in a portable format.
For app-specific preferences stored on your device, you can change or delete them directly in the app or by removing the app from your iPhone.
Changes to this policy
We may update this Privacy Policy from time to time. We will post the revised policy on this page with an updated effective date. Material changes to partner reporting practices will be reflected here and, where required, in the app before new collection begins.
